How To Get Started
Completion of the questionnaire and consultation does not guarantee evaluation scheduling.
Determination is made collaboratively.
Share your child’s history and current concerns so we can understand your needs.
We’ll review your concerns, answer questions, and determine whether a comprehensive evaluation is appropriate.
If an evaluation is recommended, we’ll guide you through next steps and available appointments.

Frequently Asked Questions
What does the parent interview portion look like?
The parent interview will be scheduled prior to in person testing and will be conducted via phone or tele-health with your
evaluating provider.
Do I need a referral?
Yes, in the state of WV-we require a referral from your pediatrician for an evaluation
How long does the evaluation take?
In-person; 45-60 minutes
How soon will I receive results?
A follow up parent meeting will be scheduled with the evaluator 1-3 weeks post testing
How soon can we schedule?
Scheduling is available on Thursday's and Friday's with availability of schedule.
Do you accept insurance?
No, not at this time-we are a private service.
We can offer you a
super bill to provide to your insurance for the possibility of
reimbursement.
UPward KidZ is a Hope Scholarship Provider. If you are eligible for Hope Scholarship please inquire about how to use these funds toward your child's evaluation.
What is our process?
Connect with us to schedule. At the time of scheduling, we will set up a time for the parent interview and in person testing. A follow up review meeting will be held where you receive the results and can ask questions!
What is the cost of the evaluation?
Evaluation fees are discussed during the consultation process, as costs may vary depending on the scope of assessment needed.
We are committed to transparency and will review all fees before scheduling.
